The Community Health Program (CHP) seeks to increase our community members quality of life through prevention, education, and coordination of care. Partnering with UCHealth, we have created a program that will help members of our community who may not have access to services or may not know about services that could elevate their well-being.
The CHP team targets clients with a wide variety of needs, including but not limited to, care coordination with area partners who aim to address housing and food insecurity, unhoused outreach, mobility and home safety for disabled and aging clients, vision and hearing needs, and behavioral health and substance use disorder.
The CHP sees clients from several sources, focusing on PFA/UCHealth online personnel referrals first. They will take clients from a high utilization list from UCHealth, if client referral load permits. CHP will also accept referrals from outside agencies when they also coincide with PFA utilization. CHP team client list considerations:
- PFA referrals/UCHealth Ambulance Crews
- PFA/UCHealth high utilizers
- Other agency referrals
The team consists of at least two personnel when visiting and resourcing clients. The CHP uses a whole person approach when establishing patient care. The team addresses behavioral health and substance use disorders, failure to thrive, assists with vitals and medical assessments, and triages any additional concerns.

The team is comprised of a manager, an emergency medical technician (EMT), and a firefighter/EMT. They visit members of our community to assess their current situation and act as a bridge for community resources, medical care, and other interventions.
