Emergency Response Survey

If you recently received a survey from Poudre Fire Authority in the mail, you can now complete the survey on the Poudre Fire web site. The Response Survey was mailed to you because our records indicated that you have recently used our services.

Please take a moment and complete this survey. Your feedback is very important to us. This information is used to help us know if we are meeting your expectations or if there is need for improvement.

To complete the Response Survey on the Web, please do the following:

1. Enter the date that your incident occurred in the first box provided on the Web survey form.

2. Answer the survey questions by clicking on your selected rating. Make sure you make only one selection for each question. If the question does not apply to the service we provided you, you may leave it blank or select “N/A.”

3. If you would like to make further comments related to the service we have provided to you, you may type your comments into the section provided at the end of the survey.

4. When you have completed the survey, click on the “Submit Survey” button to email your Response Survey to us.
