SurveysBusiness Fire Safety Inspection SurveyFIRE SAFETY INSPECTION SURVEY The Business Fire Safety Inspection Customer Survey, implemented in 1998 is now available on the World Wide Web. Every business served by the Poudre Fire Authority receives a Fire Safety Inspection. Each year over 3000 fire safety inspections are conducted by Firefighters and the Fire Prevention Bureau. At the conclusion of each fire safety inspection an Inspection Form, Fire Safety Tips, a Safety Checklist and a Postage Paid Survey Card (“Please Tell Us How We Did Today”), is left with the business. The Inspection Form serves as a record of any fire safety hazards found during the inspection and the other information provides important fire safety information to assist the business owner/manager and employees in having a fire safe workplace. Businesses are encouraged to fill out the survey card and return it to the Fire Prevention Bureau. By telling us “How We Did Today,” we can keep the service provided to you during these important Fire Safety Inspections at the highest quality level possible. This information also helps us with our continuous quality improvement efforts. To complete the “How We Did Today” survey card on the WEB, go to “SERVICES,” click on “SURVEYS,” and select “Business Fire Safety Inspection Survey.” If you would like us to contact you about your inspection, please provide us your name, phone, or email in the space provided at the bottom right of the survey form. The Fire Marshal will contact you directly. When you have completed the survey, CLICK on the GREEN Button at the bottom of the form. To reset the form, CLICK on the RED button. Emergency Response SurveyEMERGENCY RESPONSE SURVEY ON the WEB If you recently received a survey from Poudre Fire Authority in the mail, you can now complete the survey on the WEB. The Response Survey was mailed to you because our records indicated that you have recently used our services. Please take a moment and complete this survey. Your feedback is very important to us. This information is used to help us know if we are meeting your expectations or if there is need for improvement. To complete the Response Survey on the WEB, do the following: 1. Enter the Incident Number into the WEB form from the box located at the bottom right titled “For Office Use Only” into the space marked “Incident #.” 2. Enter the date that is found in this same box into the space provided on the WEB survey form. 3. Answer the survey questions by clicking on your selected rating. Make sure you make only ONE selection for each question. If the question does not apply to the service we provided you, you may leave it blank or select “N/A.” 4. If you would like to make further comments related to the service we have provided to you, you may type your comments into the section provided at the end of the survey. 5. If you would like to start over, you may click on the “RESET” button to clear the form. 6. When you have completed the survey, click on the “SUBMIT” button to send your Response Survey to us. Technical Services Customer Service SurveyPoudre Fire Authority Fire Prevention begins a new Customer Service Survey. This survey is part of our continuous effort to improve the services we provide to the community. Starting March 1, 2005 surveys will be provided to customers that have received services such as Building Plan Reviews, Site Reviews, Development Reviews, Hazardous Materials Management Plan, Fire Sprinkler Systems and Fire Alarm Systems. The above examples are only a few of the services provided by the Technical Services Program in Fire Prevention. This new survey will join the other surveys already in place; the Emergency Response Survey and the Fire Safety Inspection Survey. Information gathered from this important connection with our customers assist us with our commitment to provide the best service possible. We value your comments….Please keep your comments coming! Customers who interact with Fire Prevention Technical Services will have three options to complete this critical survey. (1) Complete the survey which will be attached to final plan reviews and/or permits by the technician that provided the service. (2) Complete and submit it on this Web site (click on the link below). (3) Take the survey with you and return by mail – no stamp required.
